Renew Home Health is hiring a compassionate home health admit RN in Fort Worth, Texas. Your main focus is new admissions and readmissions - completing the start-of-care visits that set the foundation for each patient's home health journey. From time to time, you may also be asked to help out with routine visits. Because the first visit shapes everything that follows, your work matters from day one.

What you'll do as a home health admit RN

As a PRN admit nurse, your primary focus is new admissions and readmissions across the Fort Worth area, with occasional routine visits when the team needs extra coverage. Typically, your work includes:

  • Completing thorough start-of-care and resumption-of-care assessments with OASIS documentation
  • Establishing the initial plan of care with the physician
  • Educating patients and families at the start of their care
  • Coordinating with the clinical team for a smooth hand-off to ongoing visits
  • Stepping in on routine visits when needed
  • Documenting each visit accurately and on time

Moreover, you work independently while a supportive team backs you up.

What we are looking for

First, you need an active Texas RN license. In addition, we look for the following:

  • Home health experience with admissions and OASIS / start-of-care (strongly preferred)
  • A valid Texas driver's license and reliable transportation
  • Strong assessment, communication, and time-management skills
  • Availability for PRN / as-needed visits
  • A caring, patient-first attitude
