Center Well Home Health is seeking a bilingual Nurse Practitioner/Physician Assistant to provide high-quality in-home care to seniors and adults with chronic conditions. In this role, you will conduct comprehensive assessments, develop and manage care plans, prescribe medications, and coordinate with an interdisciplinary team to support recovery, symptom management, and independence 'll educate patients and families, ensure regulatory and documentation compliance, and use innovative tools to monitor outcomes.

Responsibilities

• Provide comprehensive in-home assessments for seniors and adults with chronic conditions
• Develop, implement, and adjust individualized care plans
• Prescribe and manage medications in accordance with state and federal regulations
• Coordinate care with nurses, therapists, physicians, and other team members
• Educate patients and families on conditions, medications, and self-management
• Document visits and clinical findings accurately in the EHRMonitor patient outcomes and escalate changes in condition as needed
• Ensure compliance with home health policies, procedures, and regulatory standards
• Participate in case conferences and interdisciplinary care planning
• Contribute to a compassionate, patient-first culture and quality improvement initiatives
