Insulin Administration Nurse; LVN

Renew Home Health is hiring a dependable, compassionate Licensed Vocational Nurse (LVN) to provide insulin and diabetic care visits for patients in their homes. In this full-time role, you keep a steady schedule of visits — administering insulin, checking blood sugar, and keeping each patient safe and well-managed between physician visits. • Visit assigned patients in their homes to administer insulin as ordered
• Check and record blood glucose, and watch for signs of high or low blood sugar
  
  Follow each patient’s physician orders and plan of care
• Teach patients and families about diabetes management and warning signs when appropriate
• Document each visit accurately using our AI-assisted charting tools and report any changes to the care team
• Coordinate with the office on scheduling and route

• Active Texas LVN license in good standing
• Experience with diabetic care and insulin administration (home health experience a plus)
• A valid Texas driver’s license, reliable transportation, and current auto insurance
• Strong assessment skills and sound clinical judgment
• A caring, patient-first attitude
