Job Description & How to Apply Below
- Monday-Friday 8-5, 36 hours patient care, 4 hours admin time.
- 18 PPD, minimum is 18 PPD
- IM accepted, peds not required but preferred.
- Chart closure within 72 hours
- Chronic Pain Management experience, preferred.
- A decision is typically made within 7-14 days
- We will also accept DO
- APC's with 3 years of experience in similar setting
- The following immunizations are required for credentialing (before start date):
Flu Vaccine, TB Status/X-Ray Results (results must be within the 90 days before start date), Positive MMR Titer or Complete Immunization Series, Positive Hep B Titer or Complete Immunization Series, Positive Varicella Zoster Titer or Complete Immunization Series, Tdap Immunization (must be within the last 10 years), Covid-19 Vaccine Series and 1 Booster
