We will only review providers who will work nights. If they are strictly days then we will not be able to name clear them.

8 - OT applies after: 12 Although no OT is expected and should be pre-approved by the medical director.

- Candidate MUST have an ACTIVE CA license. Looking for 1-3 providers depending on availability Must be BC or BE within 5 years. ABEM/ABEOM certified (Will not accept ABIM/ABFM) Average number of patients in 24hrs is 110. o All HBS specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.

Please note the following travel deviations from our contract will apply to the assignment (or current confirmation agreement):
• NOT reimbursed: o Mileage from provider s home to departing airport, as well as from airport to home upon return. o Ride share (Lyft/Uber/etc.) and/or taxi from provider s home to departing airport, as well as from airport to home upon return.
• WILL be reimbursed: o Airport parking at provider s departing airport if provider drives themselves to the airport. o Insurance policies for rental cars must be pre-approved, prior to booking, otherwise it will not be reimbursed. o Ride share (Lyft/Uber/etc.) and/or taxi from destination airport to hotel/lodging for assignment, as well as from lodging to hospital/clinic, and return trips.
