Humboldt Senior Resource Center is seeking a Nurse Practitioner to provide comprehensive primary care to Redwood Coast PACE participants. You will perform initial and quarterly assessments, develop Plans of Care with the Interdisciplinary Team, and coordinate with other providers to ensure seamless care.
The role includes home visits, clinic coverage, on-call after hours, and supervision of students/volunteers.
