JOB SUMMARY
The Licensed Vocational Nurse (LVN) provides direct nursing care and care coordination for participants in the PACE program. Working under the direction of the Director of Nursing, the LVN monitors participant health status, administers medications and treatments, performs skilled nursing tasks, and responds to medical needs and emergencies. The role collaborates closely with Primary Care Providers, caregivers, and the interdisciplinary team to ensure comprehensive, participant‑centered care, and is responsible for accurate documentation, regulatory compliance, participant and caregiver education, and creating a safe care environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Monitors general health status of participants daily and administers treatments per Primary Care Provider orders.
- Communicates health concerns and recommendations to Primary Care Providers and caregivers.
- Monitors participant nutritional status and refers to the Registered Dietitian when appropriate.
- Monitors, administers, and records prescribed medications per Title XXII Regulation 54323 and 78317.
- Performs scheduled and as‑needed home visits to assess health status, provide skilled nursing care, and reinforce care plans.
- Assists participants with personal care needs, meals, and other general care duties.
- Manages medical emergencies.
- Collects data for participant assessments to ensure timely documentation.
- Conducts ongoing wound care.
- Performs blood draws when certified.
- Administers COVID‑19 tests to participants and employees as needed.
- Administers vaccines to participants and employees upon Primary Care Provider request.
- Maintains individual participant attendance, skilled nursing care records, medication administration records, and other medical data as necessary.
- Files written and verbal Incident Reports with regulatory agencies and notifies families or caregivers of incidents.
- Provides other nursing services as directed by the Director of Nursing.
- Consistently meets or exceeds Redwood Coast PACE targets for productivity and service.
- Assists the Director of Nursing in updating policies and procedures.
- Coordinates follow‑up with participants after pharmacy review and reports back to Primary Care Providers.
- Provides regular contact with pharmacists and facilitates medication delivery system review.
- Provides training and evaluation of competencies for Care Partners within the home‑care team.
- Monitors safety of the work environment and staff safety practices.
- Mentors nursing student interns within LVN scope.
- Facilitates department communication regarding participant, caregiver, employee, and program issues.
- Attends weekly staff meetings, participant conference meetings, and in‑service trainings.
- Demonstrates behaviors consistent with agency values.
- Assists in on‑call triage line coverage with Primary Care Providers.
- Provides coverage for other PACE clinic sites in Humboldt County.
- Attends meetings with outside agencies, caregivers, and families; networks with other agencies on behalf of the program.
- Reinforces education to participants and families regarding medications, medical conditions, safety, and other program topics.
- Assists participants in obtaining medical services from other providers and schedules transportation when needed.
- High school diploma or GED equivalent and successful completion of a Licensed Vocational Nurse certification program.
- At least one year of experience working with a frail or elderly population.
- Current Licensed Vocational Nurse by the California Board of Vocational Nursing.
- Current CPR and First Aid certification within six months of hire.
- Valid California Driver’s License and personal auto liability insurance; must own a personal vehicle in good condition with state minimum liability coverage.
- Successful completion of a background check, pre‑employment physical, PPD skin test, and Motor Vehicle Report.
- Knowledge of practical nursing practices within LVN scope and regulations.
- Understanding of physical, social, and mental health programs in licensed health care settings.
- Interest in working with elderly and disabled adults to promote independence.
