Redwood Coast PACE with Humboldt Senior Resource Center is looking for a full-time Director of Nursing to oversee our nursing services across multiple sites. Redwood Coast PACE — a Program of All-inclusive Care for the Elderly — is a comprehensive health plan for older adults who wish to remain living in their home and community. We provide medical, social, nutritional, rehabilitation, and support services for eligible adults.

At Redwood Coast PACE, the goal is to help people stay as healthy and independent as possible. Because their health is monitored on a regular basis, many PACE participants report fewer health crises and better overall health.

JOB SUMMARY:

Responsible for oversight of Redwood Coast PACE Nursing Services across the three sites, including management, staffing, and compliance. Directly supervises Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), student interns, and volunteers to facilitate workflows and communication within the team to ensure smooth day-to-day operations across the three sites.

ESSENTIAL DUTIES AND RESONSIBILITIES:

Participant Care and Documentation Ensures all nursing services are provided to participants daily in accordance with their care plans.

Monitors, oversees, and performs RN assessments.

Monitors general health status of participants daily; consults with Primary Care Provider (PCP) notifying whenever changes in health status are observed.

Performs scheduled and as-needed home visits to assess participant health status, provide skilled nursing care, and reinforce care plans.

Monitors nutritional status of participants and refers to the Registered Dietitian (RD) as required. Works closely with the RD in overseeing daily meals to ensure compliance with dietary restrictions and in following the Care Plan for each participant.

Administers treatments in accordance with physician orders.

Monitors, administers, and records prescribed medications per Title 22 Regulation 54323 and 78317.Collaborates with Quality to improve participants feeling of safety and satisfaction.

Responsible for managing medical emergencies in conjunction with Primary Care Providers (PCPs).Coordinates and participates in on-call coverage in conjunction with PCPs.Monitors development of Nursing sections of the Care Plans for participants, initially upon enrollment and every six (6) months, specifying short-term and long-term nursing goals.

Ensures the maintenance of individual participant daily attendance, skilled nursing care records, medication administration records, and other medical data as necessary to document medical and nursing care.

Oversees documentation of the Clinic’s RNs and LVNs.Facilitates required TB screening and documentation of records for participants.

Completes Incident Reports, as appropriate.

Oversees participant’s clinical needs in the day center.

Administration & Supervision Collaborates with the Chief Medical Officer (CMO), Director of Redwood Coast PACE, and Director of Human Resources on the employee recruitment and disciplinary processes.

Completes Interdisciplinary Team discipline-specific competencies for direct reports.

Ensures and maintains adequate daily staffing across all three clinic sites.

Ensures ongoing and program-compliant in-service training and documentation for employees, contractors, and volunteers.

Ensures regulatory compliance, including CMS and DHCS expectations.

Reports to the CMO and Director of Redwood Coast PACE concerning personnel matters of the Clinic, including situations or occurrences that impact employee relations and morale.

Participates in program and department policy and procedure development.

Enforces written agency and program policies.

Works closely with department leads to identify areas for process efficiency and implement improvements. Support system improvements by contributing to the development and refinement of procedures that optimize workflow.

Ensures alignment of team efforts with organizational goals, promoting a collaborative and efficient working environment.

Monitors and analyzes team performance to identify opportunities for continuous improvement.
