CalAIM Case Manager FloaterThe CalAIM Case Manager Floater (CACMF) plays a key role in supporting residents enrolled in Mid Pen Resident Services’ CalAIM Housing and Tenancy Sustaining Services (HTSS) program by helping participants retain stable housing and address challenges that could jeopardize their tenancy. As part of the broader Mid Pen ecosystem, this role strengthens the organization’s commitment to high‑quality, person‑centered services by providing timely coverage across properties and ensuring program compliance through accurate documentation and coordination with internal and external partners.
It’s an exciting time to join because the CACMF not only supports HTSS implementation but also contributes to leadership, training, and continuous improvement efforts that enhance service delivery across the region. The role reports directly to the CalAIM Manager.
- Implement CalAIM HTSS services at assigned properties and uphold all program standards and compliance requirements.
- Provide direct case management to HTSS participants, supporting housing stability, goal setting, and access to needed resources.
- Complete assessments, documentation, and required reporting while maintaining strict adherence to HIPAA policies.
- Respond to crisis situations and carry out mandated reporting procedures as needed.
- Support continuity of care by providing coverage across properties during staff vacancies or time off.
- Collaborate with Property Management, Resident Services, Managed Care Plans, and community partners to address participant needs and improve service coordination.
- Deliver high‑quality customer service through responsive communication, needs‑based follow‑up, and partnership with external providers.
- Maintain accurate Salesforce records—including data entry, progress notes, and participant plans—and use data to inform decisions.
- Participate in team meetings, trainings, and collaborative activities while modeling a positive, solution‑focused approach.
- Contribute to special projects, working groups, and continuous improvement efforts as assigned.

- Skilled in relationship‑building, active listening, negotiation, and conflict resolution while maintaining professional boundaries.
- Detail‑oriented with strong organizational skills; able to work independently and collaboratively.
- Knowledge of common mental health conditions and interventions including harm reduction, motivational coaching, and trauma‑informed practices.
