** Position Summary
** As a
** Licensed Practical Nurse (LPN)**, you will provide direct nursing care and support residents’ overall health and well-being. You’ll work closely with the care team to ensure high-quality, compassionate care in a supportive environment.###
** Key Responsibilities
*** Deliver
** direct patient care
** in accordance with individualized care plans
* Administer medications and treatments as prescribed
* Monitor resident conditions and report changes to the clinical team
* Support and supervise
** CNAs
* * to ensure high-quality care delivery
* Maintain accurate and timely
** clinical documentation
*** Promote a safe, clean, and respectful environment
* Collaborate with interdisciplinary team members###
** Qualifications
* ** Active, unencumbered
** LPN license in South Carolina (required)**
* ** Current CPR certification required
*** Must maintain all continuing education and remain in good standing with the State Board of Nursing
* Experience with
** Point Click Care  (PCC) preferred
*** Strong clinical, communication, and teamwork skills
