Brigham and Women’s Primary Care in Foxborough is located within the Mass General Brigham Healthcare Center (22 Patriot Place, Foxborough), which provides a comfortable and convenient location for both primary care and specialty services. The Health Care Center also offers diagnostic radiology and lab services, as well as a day surgery suite.
The Primary Care practice model in our clinic is built on team-based care; meaning that not only do you receive care from your Primary Care Provider, but the entire team is involved in your care, including Nursing, Medical Assistants, Care Coordination, Social Work, and Clinical Psychology, if necessary. Most of our physician providers are teamed up with Advanced Practice Clinicians (APC), either a Nurse Practitioner or Physician Assistant.
Our model offers our patients the advantage of expanded access and allows us to deliver care in a more effective and efficient way.
- Oversee clinical and nursing operations for Primary Care practice within a multispecialty building.
- Direct work for Clinical Practice Supervisor (current staff includes: 6 Nurses, 22 Medical Assistants).
- Direct quality and process improvement within a Triad leadership model with Medical Director and Practice Manager.
Directs, plans and coordinates the service activities and staffing performance of professional nursing and auxiliary nursing personnel in rendering quality patient care. May provide direct patient care.
Essential Functions- Ensure the development, implementation, and evaluation of staff onboarding, training, and education.
- Provide ongoing coaching and performance feedback for staff.
- Ensure a positive work environment that is safe for patients, visitors and staff.
- Coordinating unit activities with other hospital departments.
- Develop and implement staffing plans.
- Performs other duties as assigned.

General OverviewThe RN Nurse Manager is a valued member of the clinical leadership team. The primary responsibility is to coordinate and expedite the clinical operations of a multi-disciplinary team, ensuring an exceptional patient experience. The Nurse Manager reacts to the immediate operational needs of the practice, supervising and directing other members of the interdisciplinary team to further the coordination and advancement of care of patients.
Develops strong collaborative relationships across roles, disciplines and departments with a focus that ensures the needs of patients are addressed. The Nurse Manager is accountable for facilitating an appropriate response to the access needs of patients in alignment with practice based resources.
- Directs, delegates and monitors quality of care delivered by the health care team.
- Functions within and across interdisciplinary teams in a manner that promotes coordination, mutual respect and timely response to the patient's health care needs.
- Recognizes the impact of systems on health care delivery. Facilitates development of workflows and pathways that ensure the coordination of care across all settings with particular attention to transitional needs.
- Facilitates huddles, promotes and models effective, respectful communication.
- Utilizes a critical thinking approach to problem solve and make decisions.
