Social Worker‑Outreach Manager – Home Health Senior Care Agency

Location: Framingham, MA
Schedule: Full‑time, in‑office or in‑field
Travel: Required – reliable car and valid driver’s license

We are seeking a dynamic and compassionate leader to join our home health senior care team. This role combines client engagement with business development, making it ideal for someone who is people‑oriented, proactive, and passionate about improving the lives of seniors and their families.

• Community Outreach – Business Development
  • Build and maintain enduring partnerships with senior care facilities (nursing homes, hospitals, assisted‑living, senior centers, etc.).
  • Represent the agency professionally and communicate the value of our care solutions.
  • Serve as account manager, strengthening relationships with existing and new facilities.
• Referral Channel Cultivation
  • Establish mutually beneficial relationships with online and phone referral sources (A Place for Mom, Jewish Family Services, Catholic Charities, etc.).
  • Respond promptly to inquiries, converting leads into clients.
• Client Engagement
  • Manage relations with existing and new clients, building strong relationships with families.
  • Maintain regular communication through text, phone, and in‑person visits to ensure satisfaction.
  • Advocate for families navigating senior care options, adjusting approaches when needs change.
• Caregiver Back‑up and Crisis Resolution
  • Act as a backup for caregivers, serving as an interim caregiver when needed.
  • Participate in phone rotation, answering client calls to support 24/7 service.
• Office Duties and Other Responsibilities
  • Assist with weekly office duties (system documentation, care plans, etc.).
  • Coordinate events, speaking engagements, presentations, seminars, and enrich the website with testimonials, blogs, and video content.

• Previous experience in Community Outreach preferred.
• Experience in a client‑facing, in‑person customer service role, ideally with account management responsibility.
• Bachelor’s degree in Social Work or related field (Master’s preferred).
• Must be intellectually curious, high‑energy, collaborative, and team‑oriented.
• Fluent English speaker;
  Spanish and Portuguese is a plus but not critical.
• Reliable transportation and willingness to travel to facilities and client homes.
