Job Summary

Assesses patients and prepares individual Master Treatment Plans, conducting monthly reviews and updates. Performs mini‑mental status exams, psychosocial assessments, facilitates both individual and group therapy, and documents all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient absent for three or more days to determine reason for absence and intended return date.

Responsibilities
  • Design a unique Master Treatment Plan for each patient based on diagnosis and issues identified in the assessments.
  • Review and update treatment plans every 30 days.
  • Perform mini‑mental status exams and psychosocial assessments on all new patients.
  • Provide group and individual therapy and prepare timely documentation in compliance with company, state, and federal guidelines.
  • Perform other duties as assigned.
  • Perform related duties as required.
Qualifications

Education: Master’s degree in Social Work or related field.

Experience:

  • 2 years of related experience.
  • Work experience may not substitute for minimum education requirement.

Licensure/Certification:

  • Currently licensed, certified, or registered to practice the profession as required by law and regulation in the state of practice or policy.
  • CPR certification.
