Signify Health, part of CVS Health®, is seeking full-time clinicians (NPs or PAs) to provide in-home health evaluations at patients' residences. Visits last about 45 minutes and include history review, physical evaluation, and optional tests per health plan.
Clinicians will join a national network, travel locally as needed, and may conduct virtual visits. A strong license, board certification, and ability to work in the Boston area are required.
