We are seeking dedicated and compassionate Licensed Practical Nurses (LPNs) who are passionate about resident-centered care to join our team at Oak Knoll Rehabilitation and Healthcare Center. What You'll Do:- Administer medications and treatments per physician orders
- Monitor resident health and report changes promptly
- Document and transcribe physician orders accurately
- Collaborate on care plans and attend interdisciplinary care meetings
- Supervise and mentor CNAs; ensure completion of care assignments
- Participate in orientation and training of new nursing staff
- Ensure compliance with infection control, safety, and resident rights protocols
- Communicate changes in resident conditions to physicians, supervisors, and families
- Support facility leadership with audits, assessments, and other nursing functions
- Licensed Practical Nurse (LPN) in the state of Massachusetts
- CPR Certification required
- 1+ year of experience in geriatric or rehabilitation nursing preferred
- Excellent written and verbal communication skills
- Proficient in Microsoft Office and electronic documentation systems
- Able to lift a minimum of 50 lbs and perform physical nursing tasks
- Ability to work assigned shifts and adapt to a fast-paced environment
