Orthopedic Home Physical Therapist - Per Diem - Long Island

JAG Physical Therapy is seeking to hire a skilled Physical Therapist specializing in orthopedic rehabilitation in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic related conditions. Our mission is to help patients regain mobility, strength, function and confidence in the comfort of their own homes. This approach emphasizes not only physical recovery, but also the emotional and social benefits of re‑engaging with the community.

We are looking for a compassionate and motivated physical therapist to join our team and make a difference in the lives of those recovering and returning to their prior level of function. Our physical therapists will be required to evaluate and treat the patients for the duration of their plan of care in their home setting with the goal of transitioning into one of our JAG outpatient orthopedic locations for continuity of care.

What You’ll Need:

• Bachelor’s or Master’s Degree required, Doctorate degree preferred

• Licensed Physical Therapist in the state practicing in

• Completed state regulated CEU requirement in appropriate state (NY, NJ)

• Experience/knowledge with an EMR software is preferred

• Excellent oral, written communication

• Strong clinical decision‑making skills

• Excellent work ethic and dedication to patient success

• Basic computer skills

• Works as a team member and individually with minimal supervision

• Maintains a positive, professional demeanor at all times

What You’ll Do:

• Educate patient on safe bed mobility, transfers and ambulation in consideration of post operative precautions

• Transfer, ambulate and perform stair negotiation with patients to determine they are safe for discharge to home

• Collects and records patient care data that follows format and guidelines of the patient care service, regulatory agencies and third‑party payers which reflects an understanding of reimbursement systems and their impact on patient care.

• This includes, but is not limited to, initial evaluation, daily notes, reassessments/progress reports, discharge plans and communication with members of the team.

• Additionally, completes daily billing and follows department guidelines for productivity.

• Prepares for the evaluation in accordance with department policies/procedures and standards of practice including obtaining patient history, exhibiting knowledge of patient diagnoses and researching unfamiliar diagnoses.

• Selects and administers tests and measures.

• Additionally, synthesizes evaluation results to formulate the diagnosis, goals, prognosis and plan of care.

• Establishes treatment plan based on evaluation results, goals and best practice. Collaborates with interdisciplinary team, patients and patient’s support system. Assess and re‑assess need for modifications to plan of care and goals.
