Roles & Responsibilities
  • Assess residents’ communication and swallowing needs using skilled clinical judgment
  • Develop individualized treatment plans that target functional, meaningful goals
  • Implement evidence-based therapy to improve communication, cognition, and swallowing
  • Collaborate closely with nursing, rehab, and interdisciplinary team members to coordinate care
  • Document progress clearly and timely in the facility EMR to support outcomes and compliance
  • Provide family and caregiver education to promote carryover and safe swallowing strategies
  • Contribute to a positive workplace culture by sharing expertise and mentoring team members
Qualifications
  • Active Speech-Language Pathologist credential (state license required; ability to obtain Indiana licensure if not already held)
  • ASHA CCC-SLP preferred
  • 1-3 years of clinical experience, ideally including skilled nursing facility or geriatric populations
  • Comfortable with skilled documentation and regulatory requirements for SNF settings
  • Strong interpersonal skills and a collaborative approach to interdisciplinary care
Knowledge & Skills
  • Clinical expertise in adult/geriatric dysphagia and cognitive-communication disorders
  • Ability to evaluate and treat speech, language, cognition, and swallowing disorders
  • Proficient documentation skills and familiarity with electronic medical records
  • Skilled at family/caregiver education and discharge planning support
  • Efficient time management and ability to prioritize caseload in a busy SNF
