Interim Health Care seeks a Home Health Physical Therapist to provide PT services to adult and geriatric patients in their homes across the Columbus, IN area. Responsibilities include comprehensive evaluations, developing and updating individualized care plans, delivering evidence-based interventions, home safety and fall-risk assessments, and clear patient and family education. Collaborate with nurses and other clinicians, complete accurate EMR documentation, and support a patient-first culture focused on dignity, independence, and quality of life.
