Sevita is hiring Direct Support Professional Caregivers to provide person-centered care to individuals with diverse abilities in community and home settings. In this role, you will assist with daily living activities, personal care, medication reminders, transportation, and community integration. You will implement care plans, document services, and support behavioral and emotional needs while promoting independence, dignity, and safety. Responsibilities
- Provide hands-on support with activities of daily living, including bathing, dressing, grooming, and meal preparation
- Follow individualized care and behavior support plans to promote independence and person-centered goals
- Offer medication reminders and observe/report changes in health, behavior, or mood to supervisors
- Support community integration by accompanying individuals to appointments, activities, and social outings
- Ensure a safe, clean, and respectful living environment, following infection control and safety protocols
- Accurately document services, progress notes, and incidents according to agency and regulatory standards
- Use positive behavioral support and de-escalation techniques to manage challenging situations
- Collaborate with families, supervisors, and interdisciplinary team members to coordinate care
- Participate in required training, coaching, and professional development to maintain skills and certifications
