Join our dynamic team at Wellness Ranch! Wellness Ranch, a proud member of the Quadrant Health Group, is seeking passionate and skilled Full Time Case Manager to join our growing team. The ideal candidate provides care coordination and support services for clients receiving treatment within the Wellness Ranch residential substance use disorder treatment program.
What You'll Do:
The Case Manager assists clients in accessing community resources, coordinating services, and preparing for successful transition and continued recovery after discharge.
The Case Manager collaborates with clinical, medical, and behavioral health staff to support treatment goals and ensure continuity of care for clients receiving services at ASAM Levels 3.5 and 3.7/3.7-WM.
Major Tasks,
Duties and Responsibilities:
Care Coordination
• Assist clients in identifying needs related to housing, employment, education, legal matters, and recovery support services.
• Coordinate referrals to community providers and recovery resources.
• Assist clients with scheduling appointments and accessing services during and after treatment.
• Support the development of individualized discharge and continuing care plans.
Discharge Planning
• Develop discharge plans in collaboration with the clinical team and the client.
• Coordinate referrals to outpatient treatment, sober living, recovery programs, and other supportive services.
• Assist clients with obtaining necessary documentation and resources needed for a successful transition.
• Maintain communication with referral sources and community partners when appropriate.
Client Support
• Provide supportive guidance to clients navigating recovery-related resources.
• Assist clients in completing applications for housing, employment, education, and benefits when appropriate.
• Support clients in identifying and addressing barriers to recovery.
Documentation
• Document case management services and client interactions in the electronic medical record (EMR) in accordance with program policies.
• Maintain accurate records related to referrals, discharge planning, and care coordination activities.
• Communicate relevant information to the treatment team as needed.
Collaboration with Treatment Team
• Participate in multidisciplinary treatment team meetings.
• Coordinate care with clinicians, nursing staff, and other program staff.
• Communicate client needs and resource needs to the treatment team.
• Support implementation of individualized treatment and discharge plans.
Regulatory and Compliance Responsibilities
• Maintain client confidentiality in accordance with HIPAA and 42 CFR Part 2.
• Follow program policies and procedures related to documentation and client services.
• Support compliance with Kentucky behavioral health and substance use disorder treatment regulations.
• Participate in required staff training and continuing education.

• Ability to work effectively within a multidisciplinary treatment team.
Minimum Qualifications
• Bachelor’s degree in Social Work, Human Services, Behavioral Health, or related field preferred.
• Experience working with individuals with substance use disorders, behavioral health, or social services preferred.
