Responsibilities:

• Provide social work services to patients and families, including assessment, treatment planning, and counseling
• Develop and implement individualized care plans to address the psychosocial needs of our patients
• Collaborate with the healthcare team to ensure comprehensive care and communication
• Facilitate group therapy sessions and support groups as needed
• Maintain accurate and timely records of patient interactions and care plans
• Participate in multidisciplinary team meetings and provide education to the healthcare team on social work issues
• Utilize emotional intelligence and respect to provide empathetic and compassionate care

Requirements:

• Licensed Social Worker (LSW) in the state of Massachusetts
• Master's degree in Social Work (MSW) or related field
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
