Your expertise will help guide the development of individualized care plans, ensuring that our residents receive the highest quality care and support. You will work closely with our interdisciplinary team to identify and address potential issues, and provide education and guidance to residents, family members, and healthcare colleagues.

• Conduct thorough and accurate assessments of residents' medical, functional, and cognitive status, utilizing the Minimum Data Set (MDS) process
• Develop and implement individualized care plans that reflect residents' unique needs and goals
• Collaborate with the interdisciplinary team to identify and address potential issues, and develop solutions to improve resident outcomes
• Provide education and guidance to residents, family members, and healthcare colleagues on care plan implementation and resident needs
• Participate in care planning meetings and contribute to the development of individualized care plans
• Maintain accurate and up-to-date records of resident assessments, care plans, and care delivery
