Position Overview
Reliant Medical Group, part of the Optum family of businesses, is seeking a PCP-Internal Medicine APC (NP or PA) to join our team in Milford, MA. As a member of the Optum Care Delivery team, you will play an integral role in our vision to make healthcare better for everyone.
Primary Responsibilities- Independently manage a defined panel of primary care patients, providing longitudinal, relationship-based care.
- Perform comprehensive assessments, diagnoses, and treatment of patients in an ambulatory primary care setting.
- Manage acute and chronic conditions and serve as the author and supervisor of patient care plans.
- Make clinically appropriate determinations regarding in‑office management versus referral to specialty or emergency care.
- Manage inbox responsibilities, including test results, patient messaging, medication management, and referrals.
- Coordinate care with interdisciplinary team members including RNs, pharmacists, population health, behavioral health, and specialists.
- Partner with patients and families to support shared decision‑making and whole‑person care.
- Provide consultation regarding hospitalized patients within the assigned panel.
- Support practice efficiency, quality initiatives, and standardized clinical workflows.
- Participate in a shared, site‑based call structure aligned with team‑based primary care.
- Spend less time charting with DAX AI ambient scribe technology.
- Practice efficiently with Smart Rx and robust EPIC tools, including 30+ medication templates.

Required Qualifications
- Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA).
- Must have at least 3 years of paneled APC experience.
- Unlimited licensure in the state of Massachusetts.
- Active DEA license.
- Experience working within a Team Care model.
- Fluency in EMR systems & technology.
- EPIC experience a plus.
Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonuses and is based on several factors including local labor markets, education, and work experience.
