As a Licensed Practical Nurse (LPN) on our team, you will play a vital role in delivering high-quality patient care to our residents. • Provide high-quality, patient-centered care to our residents, including administering medications, wound care, and vital signs
• Work collaboratively with the interdisciplinary healthcare team to develop and implement individualized care plans
• Monitor patient progress and report any changes or concerns to the healthcare team
• Participate in quality improvement initiatives and contribute to the continuous development of our care models
• Maintain accurate and up-to-date records of patient care and treatment
• Provide education and support to patients and their families on various aspects of their care

• Current LPN license in the state of Massachusetts
• 1+ years of experience in long-term care or acute care setting
• Basic Life Support (BLS) certification
• IV therapy certification (or willingness to obtain)
• Ability to work varied shifts, including days, evenings, and weekends
