LPN | Skilled and Restorative Care | Per Diem

Responsibilities

• Assess, treat, plan, and evaluate patient care; include assessment, treatment, care planning, and medical care evaluation.
• Triage patient phone calls following established guidelines and prescribed format for collecting data and identifying patient’s health problems or concerns.
• Document relevant health information, interventions, and outcomes into the patient electronic medical record.
• Provide health education and teaching to patients in response to identified patient needs and to promote, attain, and maintain optimum health level of patients.
• Administer prescribed medications according to the five rights of medication administration.
• Perform both physical and psychosocial assessments based on patient presentation and problem, and plan nursing accordingly.
• Complete practice and unit‑specific clinical skill competencies on an annual basis.
• Perform other nursing services commensurate with LPN experience, education, and demonstrated competencies.

• Completion of a Licensed Practical Nursing program from an accredited School of Nursing.
• Licensure required:
  New Hampshire Board of Nursing license.
• Certification required:
  Current American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers.
• Demonstrated ability to effectively and positively interact with a multi‑disciplinary staff, patients and others.
• Minimum of 24 clinical hours every 4 weeks, including at least one weekend shift and at least one shift during each of the summer, winter, and difficult‑to‑fill periods.
