• Administer prescribed medications and maintain accurate documentation in resident charts.
• Monitor residents’ conditions, perform routine assessments and vital signs, and respond to changes.
• Provide basic patient care during night shifts, including hygiene, mobility assistance, and comfort measures.
• Assist with activities of daily living and support rehabilitation goals per care plans.
• Collaborate with the charge nurse and interdisciplinary team to update care plans and communicate changes.
• Document safety incidents and ensure adherence to infection control and safety protocols.

• Active LPN license in New Hampshire.
• Graduation from an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Minimum 1 year of LPN experience in long‑term care or skilled nursing settings.
• Ability to work 3x12‑hour night shifts and meet guaranteed‑hour requirements.
