LVN ​/ LPN – LTC​/SNF

LVN/LPN Position

Seeking a qualified LVN/LPN for a long-term care/snf facility to provide high-quality patient care in a dynamic environment. The role involves supporting patient needs and ensuring compliance with facility standards.

Responsibilities:

• Administer medications and treatments accurately according to care plans.
• Monitor patient conditions and report changes to healthcare teams.
• Document patient care activities in compliance with regulatory requirements.
• Assist with admission, transfer, and discharge procedures.
• Maintain a safe and clean environment for patients and staff.
• Adhere to facility policies, including COVID-19 protocols and identification requirements.

Qualifications:

• Valid LVN or LPN license in state of practice.
• Proven experience in long-term care or skilled nursing facilities.
• Ability to follow detailed instructions and adhere to safety standards.
• Strong communication and interpersonal skills.
• Maintain current immunizations, including COVID-19 vaccination, or valid medical/religious exemption.
• Must bring state or federal issued  wear a photo badge during shifts.
