Registered Nurse – LTC​/SNF

Registered Nurse

Provide skilled nursing care to patients in long-term care and skilled nursing facilities, ensuring compliance with healthcare standards and enhancing patient well-being. This role involves direct patient care, medication administration, and collaborating with healthcare teams to develop treatment plans.

Responsibilities:

• Assess patient conditions and develop individualized care plans.
• Administer medications and treatments accurately in accordance with physician orders.
• Monitor patient progress and document medical information thoroughly.
• Work closely with interdisciplinary teams to coordinate patient care.
• Provide support to patients and families, addressing concerns and education needs.
• Ensure compliance with facility policies, safety protocols, and infection control standards.

Qualifications:

• Valid RN license in the state of employment.
• Proven experience in long-term care or skilled nursing settings preferred.
• Ability to work independently and as part of a healthcare team.
• Strong communication and clinical assessment skills.
• Certification in Basic Life Support (BLS) required; additional certifications may be preferred.
• Meets all state and federal licensing and certification requirements.
