Registered Nurse

Mountain Ridge Center, a Genesis Health Care skilled nursing facility in Franklin, NH, is seeking a compassionate Registered Nurse to deliver resident-centered care. The RN will complete assessments, administer medications and treatments, support short-term rehab and long-term care residents, and document accurately in the EHR. You will collaborate with an interdisciplinary team, supervise CNAs, and uphold infection control and safety standards.

Responsibilities

• Provide direct nursing care to short-term rehab, long-term care, and memory care residents
• Perform comprehensive resident assessments and monitor changes in condition
• Administer medications and treatments per provider orders and facility protocols
• Develop, update, and implement individualized care plans
• Document assessments, interventions, and outcomes accurately in the EHRCollaborate with interdisciplinary team members to optimize resident outcomes
• Educate residents and families on conditions, medications, and care plans
• Supervise and support CNAs and other support staff during assigned shifts
• Maintain infection control, safety, and quality standards
• Participate in ongoing training, competency development, and quality improvement initiatives
