Licensed Nursing Assistant

Mountain Ridge Center, a Genesis Health Care skilled nursing facility in Franklin, NH, is seeking a compassionate Licensed Nursing Assistant to join our resident-centered care team. In this role, you will provide hands-on support with bathing, dressing, mobility, toileting, feeding, and comfort measures while accurately reporting changes to the nursing staff. You'll help maintain a safe, dignified environment, support short-term rehab and long-term care residents, and collaborate closely with nurses and therapists.

Responsibilities

• Provide hands-on assistance with bathing, dressing, grooming, toileting, and mobility for residents.
• Measure and record vital signs, intake and output, and promptly report changes to the nurse.
• Support residents with safe transfers and ambulation using lifts and other safety equipment.
• Assist with meals, hydration, and basic restorative and comfort care activities.
• Maintain clean, safe resident rooms and follow infection control and safety procedures.
• Document care provided in electronic or paper records per facility standards.
• Support residents in short-term rehab, long-term care, and memory care settings.
• Communicate respectfully with residents, families, and the care team to support resident-centered care.
