Job Summary:

Provide nursing care in long-term care or skilled nursing facility settings as an LVN/LPN. Ensure compliance with healthcare regulations while delivering quality patient support.

Responsibilities:

• Administer medications and treatments accurately per physician orders.
• Monitor patient vital signs and report changes to nursing staff.
• Document patient care activities thoroughly and promptly.
• Collaborate with healthcare team to develop and implement care plans.
• Maintain patient safety and hygiene standards at all times.
• Follow facility protocols and adhere to infection control procedures.

Qualifications:

• Hold an active LVN or LPN license valid in the state.
• Demonstrate a minimum of 1 year of experience in LTC/SNF environments.
• Complete a current immunization record, including COVID-19 vaccination or exemption documentation.
• Exhibit strong communication and interpersonal skills.
• Possess understanding of long-term care best practices and safety protocols.
