Job Summary:

Provide skilled nursing care to residents in long-term care or skilled nursing facilities, ensuring a safe and comfortable environment. Administer medications, monitor patient conditions, and collaborate with the healthcare team to deliver quality patient care.

Responsibilities:

• Assess residents’ health conditions and respond appropriately to changes in status.
• Administer medications and treatments in accordance with facility policies and state regulations.
• Document patient care activities accurately in electronic health records.
• Communicate effectively with residents, families, and interdisciplinary team members.
• Follow infection control protocols, including COVID-19 vaccination and safety procedures.
• Maintain compliance with all licensing, certification, and facility requirements.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1 year of experience in long-term care or skilled nursing settings.
• Ability to communicate clearly and work effectively within a team.
• Demonstrated knowledge of resident care regulations and safety protocols.
• Proof of COVID-19 vaccination or approved exemption documentation.
• Strong clinical skills and attention to detail.
