LVN​/LPN – LTC​/SNF

Job Summary:

We are seeking a dedicated Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) to provide quality patient care in a long-term care or skilled nursing facility setting. The successful candidate will support patient health needs while adhering to facility policies and regulatory guidelines.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor patient conditions and report changes to the healthcare team.
• Document patient care activities thoroughly and in a timely manner.
• Assist with daily living activities to ensure patient comfort and safety.
• Maintain a clean and safe environment in compliance with health standards.

Qualifications:

• Hold a valid LVN or LPN license in the state of employment.
• Demonstrate experience in LTC/SNF or similar settings preferred.
• Complete your COVID-19 vaccination or hold an approved exemption.
• Possess strong communication and interpersonal skills.
• Ability to follow detailed instructions and maintain patient confidentiality.
