Shift Details:

Rotating — 7:00 AM – 7:30 PM

Work Location:

Crisis - Franklin County Crisis Care Center;
Columbus, OH

Salary/Hourly Wage:

$19.00 - $22.00/hour

Specialties / Licenses Or

Certifications:
  • Bachelor’s Degree in a behavioral health field and a minimum of one (1) year of behavioral health experience
  • Bachelor’s Degree in any field and a minimum of two (2) years of behavioral health experience
  • High School Diploma or equivalent and a minimum of four (4) years of behavioral health experience
Summary of Job:

The Care Coordinator facilitates the quality and improvement of a person’s experience of care, which may include admission, discharge, outreach, engagement, recovery, transition and case management.

Essential Duties And Responsibilities:
  • Guest Care & Advocacy:
  • Provides coordination of care of services and resources available internally as well as throughout the community.
  • Acts as an advocate for the needs and rights of individuals.
  • Assists individuals in daily living activities and establishing and maintaining linkages that provide education, support and independence in the community.
  • Compliance &

    Risk Management:

  • Documents relevant and required information and services assuring coordination of care as a tool for communication with other team members.
  • Keeps abreast of all federal and state regulations and laws and all Recovery Innovations policies and procedures as they presently exist and as they change or are modified.
  • Understands and adheres to Recovery Innovations compliance standards as they appear in the Code of Conduct and Conflict of Interest Policies.
  • Cross Functional

    Collaboration:

  • Coordinates or facilitates multidisciplinary team meetings to ensure services are provided in a timely manner.
  • Partners with other provider agencies in the coordination of care to help participants achieve wellness.
  • Additional Duties:
  • Performs other related duties as assigned.
