About the Facility
Ahava Therapy Services is hiring a Full‑Time PT for Franklin Wellness & Rehab
, an 83‑bed skilled nursing facility that provides both short‑ and long‑term care. Therapists at Franklin work as part of an in‑house collaborative rehab team, with strong leadership support, to help residents achieve independence and optimal function.
- Conduct thorough functional assessments and develop individualized treatment plans.
- Deliver and document evidence‑based therapy sessions.
- Supervise and instruct therapy assistants as needed.
- Collaborate with nursing staff, residents, and families to maximize functional abilities.
- Educate residents and caregivers about continuing home exercise and therapy.
- Maintain compliance with documentation, safety, and quality standards.
- Participate in interdisciplinary meetings and continuous improvement initiatives.
- Maintain professional knowledge through ongoing education.
- Bachelor’s or Master’s degree in Physical Therapy.
- Current PT license in the state of practice.
- BLS/CPR certification (current or obtained during orientation).
- Strong communication, teamwork, and organizational skills.
