Enrollment Associate – Patient Support Team (Full Time)
Position Overview: As a key member of our team, you will be the first point of contact for individuals interested in our functional medicine services. Your role will be to educate potential patients about how our programs can help them recover from chronic illness, achieve their goals in our wellness program, or join our primary care program. You’ll work with a passionate, supportive team to convert incoming leads into active patients, guiding them through the enrollment process and ensuring a smooth transition into one of our care programs.
What You’ll Do:- Lead Conversion: Manage incoming inquiries (via phone, email, online forms, and word‑of‑mouth) and enter them into our CRM system.
- Initial Contact: Reach out to potential patients, assess their needs, and provide education about how our programs can support their health goals.
- Program Enrollment: Match potential patients with the most appropriate programs, explain the benefits and costs, and help them enroll in an SFM program.
- Coordinate Care: Once enrolled, assist in scheduling initial appointments, collecting necessary forms, and ensuring all administrative steps are completed.
- Collaboration: Work closely with the clinical staff to assist with ongoing care once patients are enrolled.
- Hybrid Work: Enjoy a hybrid work schedule, with a combination of remote and in‑office days, along with meetings with the Enrollment Team and other SFM team members, monthly or quarterly in‑person meetings with the whole staff.
- Sales
Experience:
Previous experience in a sales or customer service role is required, with an ability to engage, educate, and enroll potential patients. - Passion for Wellness: A genuine desire to help others heal and improve their lives through functional medicine.
- Organizational
Skills:
Ability to manage multiple leads and tasks in a fast‑paced environment while maintaining attention to detail. - Communication
Skills:
Strong verbal and written communication skills, with the ability to clearly explain complex concepts in an approachable way. - Tech‑Savvy: Comfortable using CRM software (experience with Active Campaign is a plus), email, and other online communication tools.
- Hours: Full‑Time.
- Work Environment: Hybrid, with a mix of remote and in‑office work.
