* Job Summary:
** The Advanced Practice Provider (APP) collaborates with a multidisciplinary healthcare team and is service aligned, managing a defined patient population in the outpatient setting. This role emphasizes general, episodic, and preventive care while delivering patient-centered services. A strong foundation in evidence-based practice and education is essential, as the APP ensures adherence to clinical pathways, protocols, and fosters an environment of safe, high-quality care.
The provider will demonstrate advanced medical knowledge, clinical and technical proficiency, sound clinical judgment, effective professional communication, and timely, compliant documentation. Additionally, the provider actively engages in professional development, competency validation, and contributes to the continuous improvement of patient care standards..
** Outpatient - Walk-in Clinic (Williamson County Walk-in Clinics)
**** Position
Hours:
*** Full-Time
* Mix of weekdays and weekends
* Shifts range from 9 hours on weekends to 12 on weekdays
* ** Nurse Practitioner or Physician Assistant**
* ** 1 or more years of prior APP experience required
***
* Key Responsibilities:
*** Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care.
* In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patients’ overall care, identifying expected outcomes for diagnoses.
* Orders, performs, interprets and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
* Forms differential diagnoses and treats acute and chronic conditions.
* Prescribes therapeutic interventions for both pharmacologic, non-pharmacologic, needed to achieve expected outcomes.
* Utilizes evidence-based, approved practice protocols in planning and implementing care.
* Initiates appropriate referrals and consultations
* Provides specialty specific consultation services upon request and within scope of practice.
* Facilitates the patient’s transition between and within health care settings.
