Heritage Senior Living is seeking a compassionate, dependable, and motivated CNA / Resident Assistant Supervisor to lead and support our resident care team while remaining actively involved in providing exceptional care to our residents.

This is a hands-on leadership position for someone who enjoys caring for older adults, mentoring others, and helping a team provide consistent, resident-centered care.

What You'll Do

Reporting to the Executive Director or designee, the CNA / Resident Assistant Supervisor provides day-to-day leadership to CNA / Resident Assistants while ensuring residents receive high-quality care according to their Individualized Service Plans (ISPs).

  • Supervise CNA / Resident Assistants and help ensure residents receive compassionate, individualized care.
  • Provide day-to-day direction, support, and guidance to care team members.
  • Train, coach, and mentor new and existing CNA/Resident Assistants.
  • Reinforce expectations for resident care, documentation, customer service, safety, and professionalism.
  • Monitor employee performance and provide feedback and coaching as needed.
  • Participate in corrective and disciplinary processes in partnership with community leadership.
  • Review assignments and help coordinate workflow based on resident and community needs.
  • Monitor and audit care documentation, assignment sheets, and other required records for accuracy and completion.
  • Participate in daily care focus meetings and required training.

Provide Exceptional Resident Care

  • Read and understand each resident's ISP and ensure care is provided according to the established plan.
  • Provide hands-on assistance with activities of daily living, including bathing, grooming, oral hygiene, dressing, eating, toileting, incontinence care, transferring, ambulation, and other identified needs.
  • Assist residents with mobility and transfers using proper techniques and assistive equipment.
  • Complete scheduled rounds and wellness checks.
  • Respond promptly and courteously to resident calls, pages, and alarms.
  • Assist and escort residents to appointments, activities, and other community programs.
  • Help residents maintain the highest practical level of physical, mental, and psychosocial well-being.
  • Maintain each resident's privacy, dignity, independence, freedom of choice, and individuality.

Monitor Resident Needs & Changes

  • Observe residents for changes in condition, behavior, mobility, or other needs and promptly report concerns to the appropriate supervisor.
  • Review the 24-hour communication report at the beginning of each shift to support continuity of care.
  • Contribute observations and resident information to the service-planning process.
  • Communicate resident needs and concerns effectively with families, supervisors, physicians, pharmacies, outside providers, and other members of the care team.
  • Help ensure important information is communicated between shifts and departments.

Support Medication & Care Responsibilities

  • Assist with medication administration when appropriately trained and delegated in accordance with applicable requirements.
  • Monitor Medication Administration Record (MAR) documentation for completion and accuracy.
  • Assist with controlled-substance counts and required proof-of-use documentation as assigned.
  • Help ensure medication carts are appropriately secured, organized, and supplied.
  • Obtain vital signs and collect specimens as assigned and within applicable training and authorization.
  • Complete other resident-care responsibilities according to training, delegation, company procedures, and applicable requirements.

Promote Resident Rights, Safety & Quality

  • Understand and uphold resident rights at all times.
  • Promote an environment free from abuse, neglect, exploitation, misappropriation of property, and inappropriate restraint.
  • Promptly report suspected abuse, neglect, exploitation, resident-rights violations, safety concerns, or other reportable incidents according to established procedures.
  • Follow safety, infection-control, and standard precaution practices.
  • Help maintain a safe, clean, comfortable, and homelike environment.
