• Provide skilled, compassionate nursing care to hospice patients.
• Collaborate with an interdisciplinary team to develop and implement individualized care plans.
• Support patients and families through education, comfort, and advocacy.
• Document patient care and communicate changes in condition.

• Current RN or LPN license.
• Excellent communication and interpersonal skills.
• Hospice, home health, or long term care experience is preferred but not required.
• Valid driver’s license and reliable transportation.
• Schedule Monday
  - Friday: 8:00 AM to 4:30 PM 0.8 FTE (32 hours a week), or 1.0 FTE (40 hours a week).
• On-Call Requirement:
  This role includes on call coverage. Up to 1 shift per week, Monday
  - Thursday 4 PM to 8 AM. Up to every 4th weekend, Friday 4 PM to Monday 8 AM.

  Hit the road without the extra costs- this role includes travel within a one-hour radius, and a company car is provided for employees working 0.6 FTE or more.
• Compensation: LPN: $29.19 to $32.76 per hour, based on experience. Candidates with exceptional qualifications and relevant experience may be considered for a higher rate. RN: $40.43 to $47.04 per hour, based on experience. Candidates with exceptional qualifications and relevant experience may be considered for a higher rate.
