Main Purpose of JobThe main purpose of this full-time position will be to improve community behavioral health outcomes through early identification, engagement, and intervention with children, adolescents, and adults experiencing mental health and/or substance use disorders. The primary responsibility will be to provide comprehensive, community-based case management services designed to promote recovery, increase independence, reduce barriers to care, and prevent necessary involvement with higher levels of care whenever possible.
The case manager will provide assessment, individualized service planning, referral, resource linkage, care coordination, advocacy, and skill-building services within the home and community. Services are delivered using a person-centered, recovery-oriented approach and may be provided individually or in a group setting. This position will also work collaboratively with members of the Behavioral Health Unit and community partners, including therapists, psychiatrists, primary care providers, schools, social service agencies, law enforcement, hospitals, detention center staff, the 988 Crisis System, and other local organizations to coordinate care and improve client outcomes.
In addition to routine case management responsibilities, this position will participate in a scheduled on-call rotation to provide crisis response services during evenings, weekends, and holidays. While serving on call, this position will also respond to behavioral health crises in collaboration with local law enforcement, emergency departments, detention center staff, the 988 Crisis System, and other community partners. Crisis response duties include assessment, de-escalation, safety planning, linkage to appropriate treatment and community resources, and coordination of care to reduce unnecessary utilization of emergency departments, inpatient psychiatric facilities, and detention settings whenever clinically appropriate.
Employees assigned on-call responsibilities must remain within a geographic area that allows them to respond within 30 minutes while on-call at least one weekend per month.
Employees in this classification are required to operate State vehicles to conduct home visits, provide community-based services, transport clients when appropriate, attend meetings, and perform other duties necessary to fulfill the responsibilities of the position.
Employees must possess and maintain a valid Maryland Class C Non-Commercial driver's license throughout employment.
The selected applicant must be willing and able to travel to all Worcester County Health Department sites. This position requires participation in an on-call rotation which includes evenings, weekends and holidays.
Minimum QualificationsEducation:
Graduation from an accredited high school or possession of a high school equivalency certificate.
Experience:
One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.
- Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
- Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.
Preferred Qualifications
The desired candidate should possess:
- Experience working independently providing assessment in rapidly changing situations
- Experience exercising sound judgement in high-stress environments
