Pay Range: $45,000-$55,000
Schedule:
Monday-Friday 8:00 AM-5:00 PM

• Current C.N.A license or higher licensure
• High School Diploma or GED required
• Minimum two (2) years administrative experience; one-year staffing/scheduling and/or caregiving experience preferred.
• Proficiency in Microsoft Word, Microsoft Excel, Power Point, E-mail and the Internet.

• Monitor completion of home care tasks and shifts. Report any concerns related to attendance and performance to Home Care Manager and Personal Care Services Supervisor.
• Oversee the staffing process; develop, maintain and update the Home Care schedule one-month in advance; coordinate referrals to contracted agency when approved by Home Care Manager;
  Report use of agency staff to Home Care Manager and assist with monitoring utilization of agency staff to meet home care needs.
• Ensure participant home care schedules are accurate and up to date consistently reflecting care needs outlined in IDT approved plan of care.
• Develop and maintain a good rapport with Home Care Staff and coordinate schedules per availability provided by Personal Care Services Supervisor.

As a Program of All-Inclusive Care for the Elderly (PACE) provider, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs.
