Job Summary:

The Community Health Coordinator supports residents living in Independent Living by coordinating health-related services that promote independence, wellness, and quality of life. This role works closely with residents, families, healthcare providers, and internal team members to ensure residents receive appropriate care and support while maintaining autonomy.

Essential Functions:
  • Treats all information about residents, their condition, and family as well as personnel matters as confidential information.
  • Complies with established Corporate and Departmental policies and procedures, and maintains established standards and practices.
  • Coordinates and schedules nursing services for Independent Living residents to ensure high-quality personal care and assistance with activities of daily living.
  • Builds and maintains positive, resident-focused relationships within the Independent Living community.
  • Listens attentively to resident needs and responds with empathy, professionalism, and appropriate solutions.
  • Serves as a liaison between residents, families, physicians, and outside healthcare providers or facilities as needed.
  • Participates in routine team meetings to promote communication and continuity of care, facilitating discussions as appropriate.
  • Responds to emergency calls within Independent Living, provides care within scope of practice, and contacts 911 and/or the House Supervisor (RN) when a resident requires evaluation or interventions beyond first aid.
  • Oversees ordering, receiving, inventory control, and availability of supplies and equipment.
  • Coordinates services with outside agencies and vendors as needed.
  • Performs other functions as directed by the supervisor.
  • Qualifications:
  • Current, active Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in the applicable state.
  • Current CPR and First Aid certification required.
  • Healthcare experience, preferably working with older adults.
  • Demonstrated leadership and organizational skills, with the ability to supervise and coordinate coworkers.
  • Strong verbal and written communication skills, with the ability to communicate clearly and compassionately with residents and families.
  • Ability to exercise sound judgment, problem-solving, and decision-making skills.
  • Demonstrates empathy, patience, and respect when working with older adults.
  • Understanding of basic management, delegation, and motivational principles.
  • Must be on Homewood’s approved driving list.
  • Physical Requirements:

    This role requires frequent standing and walking throughout the campus to assess resident needs, support coworkers, and respond to emergencies. The ability to lift up to 30 pounds is required to assist coworkers with resident lifts and transfers. Must be able to respond quickly and effectively in emergency situations and manage unexpected resident behaviors.
