Nurse Practitioner (In-Home Health Risk Assessments) – Locum Tenens – Charles County, MD
Part‑time Locum Tenens Nurse Practitioner for in‑home Health Risk Assessments in Charles County, MD. Flexible schedule, mobile geriatric care in Southern Maryland.
Job Details- Weekly Estimated Pay: $767 – $852
- Employment Type:
Locum tenens / contract / part‑time - Location:
Charles County, Maryland (Southern Maryland, near the Chesapeake Bay) - Shift: 1 field shift per week
- Shift Duration: 8‑hour field day (includes travel time)
- Start Date:
April 26, 2026 - Contract Duration: 13 weeks
- Travel Requirements:
Daily travel to member homes (up to 120 miles/day, typical radius ~60 miles)
- Active, unrestricted Nurse Practitioner (NP) license in Maryland
- Board certification: ANCC, AANP, or relevant national certifying body
- Minimum 1 year of NP clinical experience (new grads not accepted)
- Experience with geriatric care, home health, or in‑home assessments preferred
- Comfortable traveling independently to member homes, including underserved and rural areas
- Professional attire or dark‑colored scrubs; bring your own patient bag with basic clinical tools
- Comfort with tablet‑based documentation / point‑of‑care testing (e.g., A1C, fit tests)
- Perform annual Health Risk Assessments for geriatric members during 1:1 in‑home visits (no follow‑up required)
- Conduct focused physical exams, medication reconciliation, and social/environmental assessments
- Complete basic point‑of‑care tests (A1C, fit tests) as indicated—no blood draws
- Document assessments and submit via the provided tablet same‑day (tablet EHR / mobile documentation)
- Communicate with members: call before arrival, still visit when possible, and leave outreach materials if member unavailable
- Maintain high quality scores, adhere to assessment review processes, and follow clinical documentation standards
- Efficiently navigate daily travel routes and adapt to last‑minute cancellations, no‑shows, and schedule changes
- Strong clinical assessment skills, attention to detail, and ability to work autonomously in the field
- Comfort with mobile healthcare, home health workflows, and working with geriatric populations
- Excellent communication, time management, and problem‑solving skills for route/logistics variability
- Commitment to patient‑centered care and health equity for community‑based members
