Ennoble Care is seeking a full-time Certified Nursing Assistant or Certified Medical Assistant to serve as an Embedded Care Coordinator within one or more of our partnered long‑term care facilities in Prince George's County, Maryland. The role is integral, on‑site, and involves working closely with providers and facility staff to ensure continuity of care and reduce hospital readmissions.
Required Qualifications- Legally authorized to work in the United States
- Active, unrestricted CNA or CMA license in Maryland
- Minimum 1 year patient‑facing experience in long‑term care, skilled nursing or transitional care setting
- Effective communication skills in person, phone, email, and Microsoft Teams
- Solid knowledge of healthcare terminology and care coordination principles
- Clinical assessment skills to identify and escalates changes in condition
- Experience with EMR systems;
Point Click Care preferred - Proficiency with Microsoft 365 (Excel, Outlook, Word, Teams)
- Excellent interpersonal skills, strong organizational and time‑management abilities
- Capacity to work independently while collaborating with multidisciplinary teams
- Ability to work full‑time in an on‑site role in Prince George's County, MD
- Ability to pass background check and drug screen
- Transportation: valid driver’s license, reliable vehicle, current auto insurance
Thursday through Monday (Tuesday and Wednesday off). Daytime hours approximately 8:00 a.m. to 4:30 p.m. EST. No evenings, overnight shifts, or on‑call requirements. Seven paid holidays annually.
Key Responsibilities- Care Coordination & Transitional Care Management
- Coordinate care using Ennoble Care’s EMR for patients in skilled nursing and rehabilitation facilities
- Facilitate transitional care management and initiate services upon hospital discharge
- Expedite time to first provider visit for new transitions
- Monitor patients, communicate changes promptly, initiate interventions, and escalates care as clinically indicated, including CCM‑billable activities
- Support care coordination to reduce avoidable ED visits and readmissions
- Collaborate daily with providers and facility staff to keep care plans current
- On‑Site Clinical Support
- Maintain consistent on‑site presence in assigned facilities
- Build relationships with residents and families to reinforce trust
- Collect vital signs and perform routine patient check‑ins
- Identify changes and communicate concerns to the clinical team
- Serve as liaison between Ennoble Care providers, facility staff, residents, families, and House calls team
- Assist in converting eligible patients to Ennoble Care’s House calls program (target 2 per day)
- Documentation & EMR Management
- Accurately document interactions, assessments, and coordination activities in the EMR
- Perform chart reconciliation to ensure accuracy between Ennoble and facility records
- Follow established workflows, documentation standards, and escalation protocols
- Support onboarding and training of new Embedded Care Coordinators and serve as a mentor
