* Job Duties:
* Provide initial consultation and follow up visits as appropriate in hospital, nursing rehab, inpatient units, outpatient Supportive Care Center or place of residence
* Assess symptom burden, and craft treatment plans to reduce unnecessary suffering.
* Participate in advanced care planning discussions including recommendations for resuscitation status with supportive documentation and billing
* Lead family meetings
* Provide coaching in behavioral change or mind-body techniques to reduce illness exacerbations.
* Respond to phone questions from patients, families or members of the interdisciplinary team, including hospice nurses, for treatment options.
* Document and bill appropriately for services.
* Conduct in-service offerings for appropriate staff and community partners to raise awareness of Supportive Care issues and criteria for appropriate patients.
* Provide on call services as assigned
* Develop and maintain relationships with the hospital and/or facility staff.
* Meet with providers in private practice to develop and maintain referring relationships.
* Authorize prescriptions through the electronic system, complete electronic death certifications, and advise nursing and referral sources as required.
* Other duties as assigned by the Chief Medical Officer or Director of Chesapeake Supportive Care## Qualifications Your
Qualifications:
* Master’s in nursing with specialization in Adult, Geriatric or Family Medicine, Oncology or closely related fields.
* Active Maryland NP License and State and Federal controlled substance registration.
* Board certification in Hospice and Palliative Care required within 2 years of employment.
* DEA and Controlled substance registration, in good standing
* Interest in education of ‘next gen’ providers such as nurse practitioner students.
* Minimum of two years’ experience in treating seriously ill adult patients.
* Demonstrated understanding of and a commitment to the philosophy of Palliative Care, Familiarity with hospice, home care and adult social services.
* Fluent in the ethical and legal issues surrounding end of life care and prescribing controlled substances.
* Excellent interpersonal, organizational, written and verbal communication skills required.
* Basic computer skills, including knowledge of word processing and the ability to provide automated recordkeeping on patients.
* Demonstrated ability to navigate an EMR
* Ability to perform the essential functions of the position with/without reasonable accommodation.
