Job Summary:

The Community Health Nurse is responsible for collecting and recording routine health data, administering immunizations, and maintaining accurate records in the Electronic Health Record (EHR). This role requires adherence to confidentiality guidelines, compliance with nursing practice regulations, and proficiency in computer use. The nurse will also participate in a multidisciplinary team, travel between sites if needed, and maintain a valid driver s license with reliable transportation.

• Collect and record routine health data, including vital signs.
• Administer state-required immunizations.
• Record data and care provided in the Electronic Health Record (EHR).
• Adhere to ****, state, and federal guidelines regarding client confidentiality and privacy rights.

• RN licensure in the State of Maryland
• Current CPR certification
