The RN Assessment Coordinator (RNAC/MDS) is responsible for scheduling, and timely and accurate completion of MDS assessments for Nursing Care Center residents from admission to discharge in accordance with established policies/procedures and State and Federal guidelines.

• Active and Unencumbered RN license in state of Maryland
• Minimum of 2 years of MDS / RN experience in a Skilled Nursing Facility/LTC setting.
• 2 years of Long-Term Care experience
• Preferably a minimum of 1 year RAI experience.
• Proficiency in ICD 10 coding experience required.
• Proven leadership/supervisory skills and strong communication with staff, residents, and visitors
• Proficient computer skills for accurate data entry (Microsoft Office preferred)

• Develop comprehensive care plans based on assessments and input from residents, families, and the interdisciplinary care team.
• Schedule and coordinate PPS/MDS assessments to ensure timely completion and accuracy in all nursing sections.
• Communicate with Therapy leadership and insurance case management partners to help optimize reimbursement levels.
• Compile data for insurance updates and manage accurate MDS data entry.
• Identify and document significant changes in resident condition for MDS purposes.
• Ensure documentation supports MDS responses; educate and support staff on documentation requirements.
• Participate in Care Plan meetings as needed and collaborate across disciplines to drive strong resident outcomes.
• Stay current on state/federal regulations and help educate the assessment coordination team on policy updates.
• Participate in QAPI and Systems Improvement initiatives, including performance improvement projects.
• Maintain regulatory compliance, professional interactions, and safety practices; elevate concerns to nursing leadership appropriately.

• Current RN licensure in state of Maryland
• RAC-CT preferred not required
