Judy Center Family Service Worker

Location:

Magnolia Elementary School Responsibilities
  • Oversee the intake of referrals from Judy Center Partnerships and childcare centers.
  • Provide overall case management for children/families; coordinate services, conduct home visits, attend IEP/SST meetings, maintain telephone logs documenting contact with families, and maintain written case management reports and file logs.
  • Recruit participants and families who have children birth through prekindergarten that are eligible for the program.
  • Perform family outreach by pursuing families, following up on leads, and advising families of available services and coordinating services as needed.
  • Maintain and evaluate data for Harford County Public Schools and the Maryland State Department of Education (MSDE).
  • Update, track, and maintain the program database; responsible for MSDE evaluation components such as identifying children participating in meals programs, family intake, referrals, and intervention, and coordinating services with partners while monitoring the effectiveness of the various programs.
  • Recommend and connect families with resources and wrap‑around services as appropriate.
  • Complete and submit Medicaid Assistance paperwork associated with service coordination for children and families participating in the Judy Center Program.
  • Develop partnerships with community organizations to support and enrich the experiences of children and families participating in the Judy Center program.
  • Monitor community and Head Start outreach initiatives for children from birth to age three.
  • Utilize a database to track children/adults receiving case management services and those who participate in events for evaluation purposes, and provide this information to the Judy Center Program Manager.
  • Maintain confidentiality of sensitive information and prepare reports using such information as needed.
  • Ensure appropriate coordination between families interested in enrolling in multiple programs (e.g., child care, Head Start, Pre‑K/Kindergarten, and school).
  • Assist families with the Child Care Subsidy (child care voucher) application process and all other available services.
  • Plan and coordinate Family Nights, Parenting Seminars, Neighborhood Festivals, and activities to increase academic growth and school readiness for young children in the school catchment area, maintaining records of families who attend events for evaluation purposes.
  • Contact families to remind them of scheduled events and encourage participation to increase family involvement.
  • Attend monthly Steering Committee meetings, quarterly statewide Judy Center meetings, and annual leadership meetings with the Program Manager.
  • Provide important information related to medical concerns to the Steering Committee and partner with health services.
  • Participate as a member of the Family Outreach subcommittee.
  • Coordinate monthly service coordination meetings to ensure wrap‑around services are being provided, no duplication of services occurs, and records/tracks referrals while assigning staff and community partners for service and follow‑up.
  • Collaborate with Head Start, local behavior specialists, teachers, and families.
  • Provide the Program Manager with a bi‑weekly case management status update.
  • Participate in continuous professional development to expand skills and knowledge of young children.
  • Perform other work‑related duties as assigned.
Qualifications
  • Associate’s degree from an accredited college or university in education, social work, or a related field.
  • Demonstrated knowledge and experience working with children and families.
  • Knowledgeable of and able to work in partnership with community agencies and programs outside of HCPS; demonstrated knowledge of and ability to work with students of varying ethnic and socioeconomic backgrounds.
