Rehabilitation Specialist - Transitional Age Youth

Overview

Rehabilitation Specialist - Transitional Age Youth (TAY) program is an innovative, time-limited 8-bed residential and psychiatric rehabilitation program in Frederick, MD. It is tailored to meet the needs of young adults ages 18-23, providing a structured, home-like environment to develop daily living skills and support successful transition to adulthood and life in the community. The program engages young adults in meaningful activities such as school, volunteer work, or paid employment, and other goal-oriented activities that promote independent living and healthy life choices.

This is a full-time evening shift position that requires working 1pm-9pm and 3pm-11pm and rotating weekends.

• Engage young people through relationship development, person-centered planning, and a focus on their futures.
• Tailor services and supports to be accessible, coordinated, developmentally appropriate, and built on strengths to enable pursuit of goals across transition domains.
• Acknowledge and support personal choice and social responsibility with young people.
• Ensure a safety-net of support by involving a young person’s parents, family members, and other informal and formal key players.
• Enhance young persons’ competencies to assist them in achieving greater self-sufficiency and confidence.
• Maintain an outcome focus in the TIP system at the young person, program, and community levels.
• Involve young people, parents, and other community partners in the TIP system at the practice, program, and community levels.
• Locate and arrange participation in appropriate services within the agency and in the community for TAY.
• Work collaboratively with families, vendors, referral sources, and multidisciplinary staff to provide quality services that meet the needs of families with special needs.
• Coordinate/implement the assessment and program planning process.
• Serve as a liaison with referral sources, school personnel, and families, including initiating and/or attending outside case conferences, team meetings, or phone contacts to facilitate a continuum of care for each youth.
• Ensure completion of written documentation according to grant and Way Station, Inc. program standards, including admission, treatment, and discharge paperwork.
• Implement and monitor quality improvement measures.
• Utilize creative strategies to engage difficult youth.
• Assist with orienting youth, new staff members, volunteers, students, families, and others as needed to the program.

The pay rate for this position is $19.31 hourly.

• Your high school diploma or a GED. Bachelor’s degree in a related field is preferred.
• Professional experience:
  Two years positively referenced relevant work experience with TAY in an educational or social service setting and/or general vocational knowledge.
• Willingness to obtain additional certifications and trainings as required.
• A driver’s license with 3 points or less and access to an insured vehicle.
• Must have availability to work days, nights, weekends, and holidays depending upon program needs.
• Computer skills:
  Familiar with Microsoft Office, proficient with keyboard input into an electronic health record.
