Human Touch Home Health is seeking a Licensed Master Social Worker (MSW) for a PRN home health position serving patients in the Fredericksburg, Virginia area.

This is an as-needed role based on patient referrals and your availability.

• Develop and implement social work care plans

• Provide counseling and emotional support to patients and families

• Assist with discharge planning and long-term care arrangements

• Participate in interdisciplinary case conferences

• Ensure compliance with Medicare documentation standards

• Active Virginia Master Social Worker (MSW) license required

• Master’s degree in Social Work

• Minimum 1 year healthcare experience (home health preferred)

• Strong knowledge of community resources and discharge planning

• Excellent communication and documentation skills

• Valid driver’s license & reliable transportation
